and all parties having received proper and timely notice; the Court having heard
testiawny and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the presises, it is
ORDERED AND ADJUDGED that the minor child(ren)
TRAKELIA TAMIKA ROBINSON, d.o.b. 7/23/77
is are declared to be the legitimate child(ren) of the Defendant

JOHN ALBERT BACON , and SANDRA ROBINSON
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
SANDRA ROBINSON shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on Auttust 11 , ].980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren) I
in the amount of $ 25.00 per week plus $2.00 statutory fee

and cashiers checks shall bear the payees mate and Social Security Number and shall

Said amount shall be remitted atonthly by the Clerk to the Department of Health and
Rehabilitative Services, child Support Enforcement Unit, 1317 Winewod Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
F hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
'the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural atother or
person-having custody of the child(ren); it is further
ORDE~FD AND ADJUIGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
