THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
CORNELIUS ANTOINE STEWART. d.o.b. 12-18-74; LINDA DeQUSHELLE STEWART, d.o.b.

is are declared to be the legitimate child(ren) of the Defendant
_EUGENE STEWART , and ETHEL JACOBS ~
the natural mother; it is further
ORDERID AND ADJUDGED that the natural mother,
ETHEL JACOBS , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERID AND ADJUDGED that commencing on August 8 , 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren) '

in the amount of $ 30.00 per biweekly , plus $2.00 statutory fee i
until the AFDC is reimbursed in the amount of $3595.46
All payment shall be made in cash. money order or cashiers check. All monev orders •
and cashiers checks shall bear the payees name and Social Security Number and shall

~ Said amount shall be remitted monthly by the Clerk to the Department of Health and

~ Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
~ hereby ordered to continue to transmit support payments received from the Defendant t
until further order of this Court or receipt of Notice to Discontinue Payments from
~ the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
c person•having custody of the child(ren); it is further
~ ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated ,
a the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordei`ed to:
