AFFIDAVIT

JOHN R. DAMS, being first duly sworn, on oath deposes and says:
1. Affiant is the JOHN R. DAMS, who, with his wife, BARBARA A.
DAMS, were Grantees in that .certain deed dated 11 August 1978 conveying to
Affiant and his wife, BARBARA A. DAMS, the following described property
being in St. Lucie County, Florida:
Lot 36, Block 390, PORT ST. LUCIE SECTION 24, according to the
Plat thereof recorded in Plat Book 13, Page 31, of the Public
Records of St. Lucie County, Florida.
2. Affiant is presently married to the said BARBARA A. DAMS and
has been continuously married to her since 11 August 1978.
3. Affiant was not a patient in Fort Pierce Memorial Hospital on
24 March 1974 or 25 March 1974, hence is not the person named as Johnny
Davis in that certain Notice of Lien dated 6 September 1974 in~favor of St.
Lucie County, Florida, and recorded in Official Records Book 231, Page
1980, St. Lucie County, Florida, Public Records, or that Amended Notice of
Lien dated 26 December 1974 and recorded in Official Records Book 234, Page
2526, St. Lucie County, Florida, Public Records.
4. Affiant's Social Security Number is 266-90-5789. Affiant
knows of his owa knowledge that the Social Security Number of his wife,
BARBARA A. DAMS, is 266-96-7133.
k 5. Affiant is not the John Davis, Identifying Number
263-32-7758, against whom a Notice of Federal Tax Lien in the amount of

i $1,610.70 was recorded on 28 February 1977 in Official Records Book 264,
Page 1341, St. Lucie County, Florida, Public Records.
