FLORIDA FARM BUREAU INSURANCE COMPANY, the Worker's
ompensation Carrier for the Employer, INDIAN RIVER TERMINAL of
the Claimant, JOHN W. MEEHAN, JR., by and through their undersigned
ttorneys, pursuant to Section 440.39 of the Florida Statutes, s
fives Notice that JOHN ~V. MEEHAN, JR., received medical, compensati n
nd all other benefits provided by the Worker's Compensation Law
for his injury received on or about March 27, 1980, within the
cope and course of his employment for his employer, INDIAN RIVER
ERMINAL, and that said Worker's Compensation Carrier, FLORIDA
ARM BUREAU INSURANCE COMPANY, has paid various money for the

forementioned benefits on behalf of the Claimant, JOHN W. MEEHAN,

R., and said Worker's Compensation Carrier gives further notice

erein that it is probable that future benefits under the Florida
~ orker's Compensation Act will be paid to the Claimant.
WHEREFORE, the above mentioned insurance carrier by and

hrough its undersigned attorneys, prays that this Notice will be
~ ecorded and the same constitute a lien upon any recovery by the

laimant for the full amount of benefits previously paid on behalf
f the Claimant under Florida Worker's Compensation Act and also

rays that said Notice shall constitute a Lien upon any recovery by

he Claimant from a potential Third Party Tortfeasor for the full
