Florida, and petitioner's post office address is the same.

2. BETTY JO EVANS is an incompetent, whose date of birth was
October 4, 1961, who is 18 years of age, and whose social security
number is 262-30-9503.
3. The nature of-the incompetent's incapacity is a learning
disability_ and unable to care for herself or her property, as
adjudged by the Circuit Court in Fort Pierce, St. Lucie County,
Florida on , 1980.
4. It is necessary that a guardian of the person and property
be appointed for the incompetent.
5. The approximate value and description of the incompetent's
property are: -
