1. The decedent, SUSAN CRISLIP O'DONOGHUE, a resident ~

~t of St. Lucie County, Florida, died on July 4, 1977, and Letters r

of Administration were issued to Petitioner on February 16,

i~ 2. Petitioner files herewith a waiver of accounting by ~
the Petitioner, as the sole survivor under the Wrongful Death '
~j Act.
i~ 3. Petitioner has fully administered this Estate by
I;
making payment, settlement, or other disposition of all claims
and debts that were presented, and by paying or making provision
for the payment of all taxes at~d expenses of administration. f
4. The amount of compensation either paid or to be
paid to the personal representative, attorneys, accountants,
appraisers, or other agents employed by the personal representa-

j tive is set forth in Exhibit A attached hereto.

S. Petitioner has filed all required Estate tax returns
~
with the Internal Revenue Service and with the Department of 1

Revenue of the State of Florida, and has obtained and filed '
with :the Court evidence. of the satisfaction of this Estate's
obligations for both Federal and Florida Estate taxes, if any. ~
6. Petitioner has made or proposes to make distribution !

of the assets of this Estate as set forth in the Schedule of
Distribution attached hereto as Exhibit B. -
