~1. This application is made by THE VALLEY NATIONAL
RANK OF ARIZONA, a national banking association, the institution
named in decedent's Last Will to act as Personal Representative.
2. Decedent died ot~ or abouC the 24th day of May, 1979,
at the age of 57 years. At the time of death, decedent was
domiciled in Maricopa County, State of Arizona.
3. Decedent left surviving the persons listed on Exhibit
A attached hereto and made a part hereof by reference, who are
the surviving spouse, children, heirs or devisees of decedent.
4. Venue for this proceeding is in this county, because ~
decedent was a. domiciliary of Che county at the time of death _
~ and property of the decedent was located in the county at the i
time of death.
S. No personal representative for the decedent's estate
has been appointed in this state or elsewhere.
- 6. Applicant has not received a demand for notice and
is not aware of any demand for notice by any interested person
of any procedings concerning decedent in this state or elsewhere. '
7.. The original of decedent's will i.s filed herewith.
8. Applicant believes that the will dated December 4,
1978, was validly executed and is decedent's last will., and after f
the exercise of reasonable diligence, applicant is unaware of
any instrument revoking the will.
9. The time for informal probate has not expired because-
