FINAL JUDGMENT OF ADOPTION

THIS CAUSE came on to be heard upon the verified Petition For
Adoption and the Court having examined the Petiton and it appearing upon the
evidence that the child is suitable for adoption, that the best interests of
the child will be promoted by the adoption and that the Petitioner is a
fit and proper person to adopt the child, and the Court being otherwise
fully advised in the premises, it is thereupon
ORDERED AND ADJUDGED, as follows:
(1) That the Petition For Adoption be, and is hereby ordered
granted and that the child known as THERESA ANN DOUGHTSN ,
born on the 2nd day of November ; 19 64 , at Long Island, New York ,
is the legal child of the Petitioner, ANGELA CORRIDORS ,
and subject to-all obligations of-a child born to .the Petitioner in lawful
wedlock. -

(2) The child is given the name of THERESA ANN CORRIDORS ,
by which name the child shall be known hereafter.
