4. Thereafter, by Judgment changing name entered
by the Circuit Court of the 19th Judicial Circuit, in and
for Saint Lucie County, Florida, being Case No. 78 757 FR, and
being styled IN RE: THE CHANGE OF .NAME OF SHERI ANN McLAUGHLIN
to SHERI ANN HUBER, the name of the minor was changed from
SHERI ANN McLAUGHLIN to SHERI ANN HUBER._ A copy of such
Judgment changing name is attached hereto .as Exhibit "A" and
incorporated herein by reference.
5, At pursuant to the aforesaid Final Judgment,
Dissolution of Marriage, the care, custody and control and_
education of SHERI ANN McLAUGHLIN, now known as SHERI ANN HUBER,
was granted to the Petitioner,~CHARLY ANN SCOTT.
6.. That your Petitioner, CHARLY ANN SCOTT, is to
enter the Navy and must find suitable shelter and supervision
and care of the minor child, SHERI ANN HUBER.
7. That the Petitioners, CHARLY ANN SCOTT, FREDERICK
E. HUBER, JR., and MADELINE D. HUBER, believe that in Order
for the Petitioner, CHARLY ANN SCOTT, to properly care for the said.
• minor child and to guarantee her health and wlefare, that-the.
said MADELINE D. HUBER and FREDERICK E. HUBER,JR have certain
rights, duties and obligations to the said minor, SHERI ANN
~ HUBER, and therefore, joins in petitioning this Court to
i appoint the aforesaid MADELINE D. HUBER and FREDERICK E. HUBER,
JR., guardians of the person of the said minor. -
8. The Petitioner, CHARLY ANN SCOTT, being the
natural mother and custodian of the minor child, SHERI ANN
HUBER, does hereby concur, consent and join in this Petition.
9. GEORGE DAN McLAUGHLIN has a no time subsequent
to November 27, 1973 paid or provided any sums of money
to or for the benefit of the care, maintenance and/or edu-
cation of the minor child, SHERI ANN McLAUGHLIN, now known as
SHERI ANN HUBER.
10. Petitioners, MADII.It~ D. HUBER and FREDERICK E, HIJB~IIt, JR.,
sotx~thereby to be appoantedas Guardians of the Person of SHERI ANN
HUBER, are qualified to act as Guardians under the provisions
