The undersigned, as personal representative of the above estate alleges:
1. The decedent, ALLIECE K. WALTERS a resident of
ST. LUCIE County FLORIDA died on -

Petitioner on AUGUST 16 ~ 19 ~ ,
2. Petitioner files herewith a waiver of final accounting executed by all
beneficiaries of this estate.
3. Petitioner has fully administered this estate, by making payment, settlement, or other
disposition of all claims and debts that were presented, and by paying or making provision for the
payment of all expenses of administration.
{ 4. Petitioner has filed all required estate tax returns with the Internal Revenue Service and
with the Department of Revenue of the State of Florida, and has obtained and filed with this court
en?idence of the sati.4faction of this estate's obligation.4 for both federal and Florida estate taxes, if any.
S. Petitioner has made or proposes to make distribution of the assets of this estate as set
forth in the schedule of distribution attached hereto as Exhibit A.
