CREDIT LIFE AND/OR DISABILITY INSURANCE IS VOLUNTARY AND NOT REQUIRED FOR CREDIT. Such Insurance
coverage is available at the cost designated below for the term of the credit. Florida law requires that before any credit life insurance
may be sold, the creditor must advise the borrower that he has the option of assigning any other policy or policies the debtor owns or
may procure for the purpose of covering said loan and borrower is so advised.
