The undersigned petitioner alleges:
1. Petitioner's residence is 207 Orange Avenue, Fort
Pierce, Florida, and petitioner's post office address is Post Office
Box 3698, Fort Pierce, Florida 33454.
2. STANLEY BALDIGA is an incompetent, whose date of birth
was February 4, 1907, who is 76 years of age, and whos: social
security number is 287-07-0557. The residence of the incompetent
is 108 North 8th Street, Fort Pierce, Florida.
3. The nature of the incompetent's incapacity is aphasia; .
paralysis on right side of body, as adjudged-by the Circuit Court
in St. Lucie County, Florida, on the ~ 3 day of ~ ,
1980. -
4. It is necessary that a guardian of the person and pro-
perty be appointed for the incompetent.
