PETITION FOR DISCHARGE
The undersigned, as personal representative of the
above Estate, alleges:

~j 1. The decedent, MIKE FAPPAS, a resident of St. Lucie ~
i~ county, Florida, died on December 7, 1979, and Letters of

~ Administration were issued to Petitioner on December 18, 1979.
2 Petitioner files herewith a final accounting con-

taining a complete report of all receipts and disbursements

since the commencement of administration of this Estate.

I~ 3. Petitioner has fully administered this Estate by
1~ making payment, settlement, or other disposition of all claims
~j ar~d debts that were presented, and by paying or making provision.

for the payment of all taxes and expenses of administration.

j 4. The amount of compensation either paid or to be

`i paid to the personal representative, attorneys, accountants,

appraisers; or other agents employed by the personal representa-

tive is set forth•in Exhibit A attached hereto.

5. Petitioner has filed all required Estate tax returns
with the Internal Revenue Service and with tiie Department of

Revenue of the State of Florida, and has obtained and filed
with the Court evidence of the satisfaction of this Estate's
obligations for both Federal and Florida Estate taxes, if any.

- 6. Petitioner has made or proposes to make distribution
of the assets of this Estate as set forth in the Schedule of
Distribution attached hereto as Exhibit B.
