PETITION FOR ADMINISTRATION
The Petitioner, LEROY 5. KOUSEK, alleges:
1. Petitioner has an interest in the above estate as
surviving spouse. Petitioner's address is 657 North 2nd Street,
Lot #63, Fort Pierce, Florida 33450, and the name and address
of the Petitioner's attorney is set forth at the end of this
petition.
2. Decedent, JEAN KOUSEK, whose last address was 709
South 33rd Street, Fort Pierce, Florida 33450, and whose age
was 65 and whose social security number is 397-222396A, died on
December 25, 1979 at Tampa General Hospital, Tampa, Florida 33602,
at the time of death decedent was domiciled in St. Lucie County,
Florida.

3. S'o far as is known, the names of the beneficiaries of
this estate, their addresses and relationships to decedent, and
the ages of-who are minors, are:

4. Venue of .this proceeding is in this County because
decedent was resident and domiciliary at the time of death.
5. LEROY S. KOUSEK, whose address is 657 North 2nd Street,
Fort Pierce, Florida 33450, and who is qualified under the laws of
tl~e State of Florida to serve as personal representative of
decedent's estate is entitled to preference in appointment as
personal representative because he is the surviving spouse and sole
heir of the decedent.
6. The nature and approximate value of the assets in
this estate are: possible settlement in wrongful death claim.
7. After the exercise of reasonable diligence, petitioner
is unaware of any unrevoked wills or codicils of decedent.
Petitioner requests that LEROY S. KOUSEK be appointed
personal representative of the estate of the decedent.
Under penalties of perjury I declare that I have read the
foregoing and the facts alleged are true, to the best of my
knowledge and belief.
