child support signed by the party leaving custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and

Said amount shall be remitted aanthly by the Clerk to the Departaknt of Health aAd
Rehabilitative Services, Child Support Enforcement Unit, 1317 Wineuaod boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received frog the Respondent.
