THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the pcemises, it is
. ORDERED AND ADJUDGED that the minor child(ren)
RODRICK DEVON FRAZIER, d.o.b. 12/13/78
is/are declared to be the legitimate child(ren) of the Defendant
RODNEY MITCHNER , and VICTORIA~FRAZIER ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
VICTORIA FRAZIER , shall Have custody of the said child(ren)
sub,~ect to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing~on October 10 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
i~n the amount of $ 2 ~ C7c~ per L~J~-Q plus $2.00 statutory fee
until the AFDC is reimbursed in the amount of $691.00
j All payment shall be made in cash, money order or cashiers check. All monrv orders
~ and cashiers checks shall bear the payees name and Social Security Number and shall

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida,. 32304; it is further
ORDERED ANU ADJUDGED that tt~e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the De{~artment of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); tt is further
~ ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DF.PARTMF.NT OF HEALTH AND REHABILITATIVE
~ SERVI.CES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
