F263-43-6262 DefE~ ~dant . / THIS CAUSE having come on for trial upon the pleadings filed herein and all parties having received proper and timely notice; the Court having heard testimony and/or considered the pleadings, papers, affidavits and other papers filed herein, and being otherwise fully and well advised in the premises, it is ORDERED AND ADJUDGED that the minor child(ren) LaSONJA TRANEE ADDERLEY, d.o.b. 9/14/72 is/are declared to be tl~e legitimate child(ren) of the Defendant RODNEY MITCHNER , and CYNTHIA ADDERLEY the natural mother; it is further ORDERID AND ADJUDGED that the natural mother, CYNTHIA ADDERLEY shall have custody of the said child(ren) subject to the Defendant s right of reasonable visitation; it is further ORDERED AND ADJUDGED that commencing on October 10 , 1980, the Defendant/Father shall pay child s~~pport for and on behalf of the said child(ren) in the amount of $ X2.50 per week , plus $2.00 statutory fee until the AFDC is reimbursed in the amount of $2111.96 . FLORIDA 33450. - ; _ Said amount shall be remitted monthly by the Clerk to the Department of Health and Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, ~ Tallahassee, Florida, 32304; it is further ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is _ hereby ordered to continue to transmit support payments received from the Defendant until further order of this Court or receipt of Notice to Discontinue Payments from the Department of Health and Rehabilitative Services, in which event the support payments shall thereafter be directed and payable to the aforesaid natural mother or person-having custody of the child(ren); it is further ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to: (Check applicable paragraph) X 1. amend the above-named child's/children's birth certificate(s) to show the - ~ above-named father's name. 2. remove from the above-naaed child's ~ children s birth certificates end enter the above-named father's name. ~ DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this 6th day of October 1980.
