BEFORE ME, the undersigned authority, personally appeared
JOSEPH J. DeROSS, JR., ESQ., and says as follows:
1. That he is the attorney for the Personal Representatives
of the Estate of ALBERT S. RUCKS, deceased.

2. That on September 30, 1980, the affiant filed an Affidavit
listing all of the beneficiaries that were to be included on the Peti- 4
i
tion for Formal Administration which was filed with this Court on
September 25, 1980.
3. That to the best of the Affiant's knowledge and belief,
each and every beneficiary was listed on said Affidavit.
4. That subsequent to the filing of said Affidavit, the
Affiant has been informed that ALBERT S. RUCKS, deceased, had one

additional niece whose name, address and age is as follows:

~ 5. It has further come to the attention of the Affiant that
two of the names listed on said Affidavit were incorrect, in that
SUSAN RUCKS LITCHER'S name is now SUSAN RUCKS MANN and that ADA i
LEMIEUX name is now ELEANOR A. JOHNSON. Further, ELEANOR A. JOHNSON'S
address is 240 Jackson Way, West Palm Beach, FL 33406.
6. It is the intention of the Affiant to have these correc-
tions and additions added to paragraph three of the Petition for

Formal Administration filed in this Court on September 25, 1980. ~
FURTHER AFFIANT SAYETH NOT.
