_ RESIGNATION OF GUARDIAN AND
PETITION FOR DISCHARGE

The undersigned Petitioner alleges: ~
1. .Petitioner is the duly qualified and acting-guardian
t
of the person and property of THOMAS E. HALL, Incompetent.
2. Petitioner wishes to resign and be relieved of his ~
duties as such guardian and therefore hereby submits his resigns-
tion as such guardian. ~
3. Notice of this resignation has been given to the ~ F
surety on petitioner's bond and there are no other interested
persons to whom notice must be given.
(
WHEREFORE, petitioner requests that appropriate orders
~ be entered:
(a).Determining what additional notice, if any, the
~ court may require.
(b) Finding that the interests of the ward will not
be placed in jeopardy by petitioner's resignation. -
(c) Accepting petitioner's resignation.
(d) Appointing a successor guardian.
(e) Relieving petitioner of his duties upon the
appointment and qualification of the successor guardian.
~ (f) Discharging petitioner and his surety from further
liability after petitioner has filed a true and correct account of
~ petitioner's guardianship and has delivered to the successor. guardian
all property of the ward, all records concerning the property of the
ward or the guardianship, and all the money due to the ward from
petitioner.
