The undersigned petitioner alleges:

1. Petitioner's residence is 302 North 14th Street,
Fcrt Pierce, Florida 33450.
2. THOMAS E. HALL is an incompetent, whose date of birth
is January 12, 1915, who is 65 years of age, and whose social
security number is 263-07-0701. The residence of the incompetent
is Easter Manor Nursing Home, 611 South 13th Street, P. O. Box 1658, ~
Fort Pierce, Florida 33450. ~
- 3. The nature of the incompetent's incapacity is a left
cerebral vascular accident, diabetes, hypertension and renal disease,

as adjudged by the Circuit Court of St. Lucie County, Florida, on {
f April 22, 1980. ~ ~

4. It is necessary that a guardian of the person and
property be appointed for the incompetent. -
5. The incompetent owns no property known to the
Petitioner.
6. There are no known persons closely related to the
incompetent.
7. Venue of this proceeding is in this county because
the incompetent is a resident of St. Lucie County, Florida.
8. JACQUELINE ALBERT a resident of St. Lucie County is
~ sui juris and otherwise qualified under the_laws of Florida to act
as guardian of the person and property of the incompetent.
