PETITION FOR VOLUNTARY GUARDIANSHIP
The undersigned petitioner alleges:
1. Petitioner's residence is 510 South 23 Street, Fort
Pierce, Florida, and petitioner's post office address is
510 South 23 Street, Fort Pierce, Florida.
2. Petitioner, whose date of bixth was the 4th day of
November, 1894, is 86 years of age, and her social security number
is 149-12-4594.
3. Petitioner, although mentally competent, is incapable .of
the care-, custody and management of her estate by reason of age or
physical infirmity and voluntarily petitions the Court for the
appointment of a guardian of her property pursuant to the applicable
provisions of the Florida Statutes.
4. This petition is accompanied by a certificate of a licensed
physician that he has examined petitioner and that petitioner is
competent to understand the .nature of the guardianship and his

delegation of authority. _
5. The a roximate value and descri tion of the

property are real .and personal property in the amount of $ 25,000.00
6. The names and addresses of the persons most closely related
to petitioner are: None. _
7. Venue of proceeding is in this county because there are
assets of the ward here and that St. Lucie County is her residence.
8. Reasonable search has been made, and any of the information
required by Florida law and by the applicable Rules of Probate and
Guardianship Procedure that is not set forth in full above cannot be
ascertained without delay that would adversely affect the petitioner
