THIS CAUSE having come on far trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
ALLEN FERGUSON, JR., d.o.b. 8/23/78 _
is/are declared to be the legitimate child(ren) of the Defendant
ALLEN FERGUSON and SANDRA FLAGG ,
the natural mother; it is further
ORDERID AND ADJUDGED that the natur.ll mother,
SANDRA FLAGG , shall have custody of the said ch'i1d(ten)
subject to the Defendant s right of reasonable visitat~i/o`n; it is further
ORDERID AND ADJUDGED that commencing on O ~ ~ 1980,
the Defendant/Father shall pay child-support for and on behalf of the said child(ren)
in the amount of $ 25.00 per month plus $2.00 statutory fee
until the AFDC is reimbursed in the amount of $675.00

All payment ahal' be made in cash. money order or cashiers check. All monev orders
and cashiers checks shall bear the payees name and Social Security Number and shall
be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

~ Said amount shall be remitted aanthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
( Tallahassee, Florida, 32304; it is further
~ ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
• hereby ordered to-continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
I the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural sother or
person-having custody of the child(ren); it is further
ORDERED A~tD ADJUDGED that the above-named Defendant havins been adjudicated
