The undersigned, as Personal Representative of the above
'Estate, alleges:
'i 1. The decedent, John A. Suarez, a resident of Monroe
:County, New York, died on the 21st day of December, 1978, and Ancil ary
;Letters of Administration were issued to Petitioner on June 11, 198 .
2. Petitioner files herewith Wavier of Accounting and
:;Service of Petition for Discharge and Consent to Discharge of An-
;`':cillary Personal Representative.
` 3. Petitioner has fully administered this estate,. by
s making payment, settlement, or other disposition of all claims and ~
'debts that were presented, and by paying or making provision for
:;the payment of all expenses of administration.

4. -Petitioner has filed all required estate tax returns
';with the Internal Revenue Service and with the Department of Reve-
~~nue of the State of Florida, and has obtained and filed with this
''Court Non-taxable Certificate No. E89869, attached hereto. (Exhibit A)
5. Petitioner has made distribution of the assets of
;;this estate as set forth in the schedule of distribution attached
~ +as Exhibit B. ~
6. The only persons having an interest in this proceed-
~~ing and their respective addresses are: ~

Petitioner requests that, after satisfactory proof has
;'been presented that distribution has been made in accordance with
~~the schedule of distribution and that claims of creditors have
°been paid or otherwise disposed of, an order be entered discharging
;2Petitioner as Personal Representative of this estate and releasing ,
;the surety on any bond which Petitioner may-have posted in this
~ :!proceeding from any further liability on it, if any.
