of the decedent. His name and address is as set forth in Paragraph 3 '
and the name and address of his attorney is as set forth at the erxl of
this Petition.
2. Decedent, I~6SLYN 9~, whose last ]uso~wn address
was 14 Cass Place, Brooklyn, New York, died an December 8, 1979 at
Brookdale Hospiital Medial Center, Brooklyn, New York and at the time
of death decedent wes domiciled in the Borough of Brooklyn, State of
New York.
3. So far as is know, the names of the beneficiaries of ~
this estate, their addresses and relationships th decedent, arr3 the -
ages of any who are minors are:
