The undersigned petitioner alleges:
1. Petitioner's residence is 1915 Mimosa Avenue, Fort Pierce,

2. LULU MAE VERDEL is an incompetent, whose date of birth was
August 10, 1896 ~ who is 86 years of age, and whose social security

(33450) and whose post office address is the same.
3. The nature of the incompetent's incapacity is Cerebral
Arteriosclerosis with chronic Organic Brain Syndrome, as adjudged
by the Circuit Court in St. Lucie County, Florida on November 12 ,

be appointed for the incompetent.
