1. Petitioner has an interest in the above estate
as the person selected by the beneficiaries under the decedent's
last will to serve as ancillary personal representative.
2. Decedent, Elizabeth Z. Smyth, whose last known address -
was 1031 Prospect Road, Ashtabula, Ohio 44004, died on June 1,
1979 and at the time of death decedent was domiciled in
Ashtabula, Ashtabula County, Ohio.
3. So far as is known, the names of the beneficiaries
of this estate, who are over 18 years of age, their addresses
and relationship to the decedent, are:
