The undersigned petitioner alleges:
1, Petitioner's residence is 310 West Giddens Avenue,
Tampa, Florida (33603) and petitioner's post office address is same.
2, Leona M, Cook is an incompetent, whose date of birth
was January 5, 1904, who is ?6 years of age, and whose social security
number is 266-20-0509, The residence of the incompetent is Habana
Villa, 2916 Habana Way, Tampa, FL (33614) and the post office address
of the incompetent is the same, temporarily. Her permanent address is
624 South 12th Street, Fort Pierce, Florida (33450).
3. The nature of the incompetent's incapacity is DX:
Schizophrenic Reaction, Undifferentiated Type, as adjudged by the Circuit -
Court in St. Lucie County, Florida on July 17th, 1980.
