The undersigned, as guardian of the property

1. The need for the continuation of this guardianship no longer exists by reason of the fact

2. Petitioner files herewith a final return containing a full and correct account of aU
receipts and disbursements of all the ward's property of which petitioner has control since the last
return filed herein, J any, or from the commencement of the administration of this guardianship if
no prior return has been filed, which return includes a statement of the ward's assets..
3. Petitioner represents that Hans John Malchow, 128 North 14t:h Street,
Fort Pierce, Florida 33450

WHEREFORE, petitioner requests that an order be entered approving all accountings not
previously approved and discharging petitioner as guardian and releasing the surety on petitioner's
