THIS CAUSE having come on for hearing on the petition of the DEPARTtlENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an asaignsent of rights to
child aupport~signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERID AND ADJUDGED that the Petition for Child Support shall be and

Said amount shall be remitted monthly by ttie Clerk to-the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Wlnewood Boulevard,
Tallahassee. Florida, 32304. It is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to contim~e to transmit support payments received Eros the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Paysents from the Department
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
