THIS CAUSE having come on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to
child support signed by the party leaving custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is licreby branted; it is further
ORDERED AND ADJUDGED that cunimencing on ~Q ~, ~ ____, 1980,
the responsible parent PHILLIP D. CHANDLER ~ who is the father - ~
of the minor child(ren), to-sit: JOHN ALLEN PHILLIP CHANDLER, d.o.b. 11/10/77;

Said aaaunt shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support EnEorceaent Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to contin-:e to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue PaymentR from the DepartmKnt
of Health and Rehabilitative Services, in t~rhich event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this
