THIS CAUSE having come on for hearing on the petition of the DEPARI?IENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assigneat of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERED AND ADJUDGED that commencing on JANUARY 20, 1981 _~ L
the responsible parent DAISIE MELTON , who ie the mots--er
of the minor child(ren), to-Wit: NETELLA PAIGE, d.o.b. 2/12/72
shall pay to the Clerk of this Court the sum of $ 30.00 per month each and
every month , plus S2.00 statutory fee, until the AFDC s rein ursed in`the
amount of $1503.00 All payments shall be made in cash,
money order or cashiers c ec 1 money orders and cashiers checks shall bear the
payees name and Social Security Number and shall Ee made payable to the CLERK OF THE
CIRCUIT CWRT, and sent to:
Clerk of the Circuit Court
Support Department
P. 0. Box 700
Fort Pierce, Florida 33450.
Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winei-ood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Departaent
of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
. DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this
