and all parties having received proper and timely notice; the Court hsving heard
testimony acd/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and Kell advised in the premises, it is
ORDERED AND ADJUDGED that the sinor child(ren)

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of tl~e Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Nottce to Discontinue Payments fros
the Department of Health and Rehabilitative Services, in which event the support
pay~uents shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNTT, shall and it is hereby ordered to:
