The undersigned does hereby apply- to the Fort Pierce Utilities
Authority for an extension of ~ea.ter service outside the City Limits of
the City of Fort Pierce, Florida, to the followinfi described property
~ehich I own, to-wit:
Lot 24, S. 15' of Lot 25 in Blk 2, of Blakely Subdivision, according
to Plat thereof recorded in Plat Book 10 at page 50 of Public Records
of St. Lucie County

I understand that in order to obtain Axtension of such water
service, that I will l;e rc~quirecl to pay the cost of construction of
such ~+~ater line from the point of connection With e:cistino lines of the
Utilities Authority to r.~y property, that the total of such cost shall
be deposited :vith the [Itilitics Authority prior to the tirn~ of the
construction of said line, that I will be obligated to pay all meter
fees and connection charges required under the Resolutions of the
Utilities Authority for such service, prior to the installation of said
line, and that I will pay, when billed, for all water furnis!Zed to my
property according to the rates established by the Utilities Authority-
for the service I receive.
In addition to the deposit of said construction cost as set
forth in the above paragraph, I understand that upon execution of this
~cpplicsltion, I am required to deposit with the Utilities Authority an
amount equal to ten (lOn) percent of the connection fees for water
service for each unit to be served to and including tv.enty-five (25)
units, and five (5p) percent of such connection fees for each unit in
excess of twenty-five (25) units. In the event this application is net
approved by the Utilities Authority and the City Cor•:mission of the City
of Fort Pierce, then such deposit will be returned to me. If this
an;~7.ication is approve, by the Utilities Authority and the City Com-~issioa
c)f tht< City of Fort Pierce, the deposit of the connection fees as above
