This cause was duly presented by the State Attorney and the Respondent having been present in
Court, and the Court being advised in the premises, it is,
ORDERED AND ADJUDGED that the Respondent pay to the Petitioner for the benefit of
EPHRAIM WEST, JR., born 3/8/74,
and LOUIS E. WEST, born 9/4/78*
RANDY E. WEST, born 11/14/77;
minor children, the sum of ~515. QO per weekiper chilc~~us and additional 3k~8x $2.00
for payment of Clerk's fee for receiving and disbursing support payments.
These payments are to begin on the
2nd day of January , ~9 81 , and thereafter on Friday
of each and every ~k that said payments shall be made in cash, by cashier's check
or by postal money order to the Clerk of the Circuit Court of St _ LuciP County,
P. O. Drawer 700, Fort Pierce, Florida
Florida,/nd the said Clerk shall transmit the said payments to Child Support Enforcement
Section, 1317 Winewood Blvd., Tallahassee, Florida 32301, for transmittal
to the State of Pennsylvania as long as the case is certified as a Title
IV-D case. If the Clerk receives notification by the Department of Health
& Rehabilitative Services that the case is no longer a IV-D case, he will
