AND ACCEPTANCE
The petitioner, MARTHA E. PANAKOS, alleges:
l. Petitioner has an interest in the above estate
as the named personal representative. Petitioner's name
and address are: MARTHA E. PANAKOS, 2108 N. E. 44th Street,
Fort Lauderdale, Florida, and the name and address of Peti-
tioner•s attorneys are as set forth at the end of this
petition.
2. Decedent, Lyman B. Curtis, whose last known ad-
dress was 67 Pleasure Avenue, Fort Pierce, Florida, whose age
was 62 years, and whose social security number is 364-28-7152,
died December 9, 1980, at Sebastian, Florida, and at the time
of death decedent was domiciled in St. Lucie County, Florida.
3. So far as is known, the names of the beneficiaries
of this estate, their addresses and relationships to decedent,
and the ages of any who are minors, are:
