1. Petitioner has an interest in the above Estate as
the former wife of the decedent. Petitioner's name and address

' 2. Decedent, JOHN W. HENRICKS, whose last known address

;` was 502-B South 22nd Street~ Fort Pierce, Florida 33450~ whose
. age was 34 years, and whose Social Security P~umber was
~ 267-70-7985, is presumed to have died in a plane crash on
' August 7, 1978~ in the vicinity of Great Inagua. Pursuant to
:§731.103(2), Florida Statutes, a copy of the United States
; Coast Guard Seventh District Case No. N 669 SA report is attached
hereto as prima facie evidence of the date~ circumstances, and
the place of presumed death.
