That I am the survivinq spouse of ARNOLD H.
HENRZKSEN who died November 1, 1979; that said
decedent and I were married on April 24, 1948 in the
City of Rutherford, State of New Jersey, and have
continuously lived as husband and wife from the date
of marriage to the date of his demise.
I declare, under penalty of perjury, that the
foregoing is true and correct.
