as follows:
1. Petitioner resides at 1020 South 15th Street. Fort
Pierce, St..Lucie County, Florida, 33450. and her post office
address is the same.
2. GABRIEL M. ANDERSON is a minor, born the 16th day of
November. 1972, whose age is 8 years, whose residence address
is 1020 South 15th Street, Fort Pierce, Florida 33450, and is
the son of your Petitioner.
3. The minor is the owner of a personal injury claim
against EDNA C. DEILY and the HARTFORD ACCIDENT AND INDEMNITY
COMPAt1Y, the approximate net value of which equals $1~,750.00
(~aelve Thousand seven Hundred Fifty Dollars).
4. The name and address of the person most closely
related to the minor is as follows:
Shirley S. Hotchkiss. mother
1024 South 15th Street
Fort Pierce, Florida 33450
5. The mother and father, MICHAEL ANDERSON. were divorced
and the Final Judgment of The Dissolution of Marriage was
entered awarding the custody of said minor child to the natural
mother. A copy of the Final Judgment of Dissolution of Marraig~.
