The Petitioner, CAROL M. BERG, alleges:
l. Petitioner has an interest in the above estate as
beneficiary under the Last Will and Testament of decedent and
petitioner has been appointed executrix of the estate in Frank-
lin County, Ohio. Petitioner's name and address are Carol M.
Berg, 785 Chelsea, Columbus, Ohio ~3209, and the name and
address of petitioner's attorney are as set forth at the end of
this petition. .
2. Decedent, JOHN J. FEYES, whose last knawn address was
2946 Berwick Boulevard, Columbus, Ohio 43209, and if known,
whose age was 66 years of age and whose social security number
is 282-03-3258, died on November 21, 1979, and at the time of
death decedent was domiciled in Franklin County, Ohio.
' 3. So far as is known, the names of the benficiaries of
this estate, their addresses and relationships to decedent, and
the ages of any who are minors, are:
