JUDGMENT CHANGING NAME
THIS CAUSE having been heard before me upon the sworn Petition
of BETH ANN MEDLEY, as Parent and Natural Guardian of JAY ANDREW
VORNAUF, a minor child, and the Answer and Waiver of RICHARD E.
VORNAUF, the natural father of said minor child, for a Judgment
changing the name of JAY ANDREW VORNAUF to JAY ANDREW MEDLEY; and it
appearing to the Court that Petitioner is an actual bona fide resi-
dent of St. Lucie County, Florida, and the Court being otherwise
fully advised in the premises, it is
ORDERED AND ADJUDGED:
1. That the Petition is granted.
2. That the name of JAY ANDREW VORNAUF is changed to
JAY ANDREW MEDLEY.
DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida,
