PETITION FOR APPOINTMENT OF GUARDIAN
The unders-igned petitioner alleges:
1. Petitioner's post office address and place of
residence is 464 Brattle Road, Syracuse, New York 13203.
2. JESSE I, BAGGETT is an incompetent, whose date
of birth was March 13, 1909 , who is 71 years of age, and whose
social security number is 240-IO-3823. The post office atidress
and pYace of residence of the incompetent is 900 Seager Avenue,
Fort Pierce, Florida 33450.
3. The said incompetent is presently confined in the
Abbiejean Russell Care Center, 700 S. 29th Street, Fort Pierce,
Florida ~33450.
4. The nature of the incomPetent's incapacity is out-
lined on a copy of the Psychiatric Evaluation attached hereto
and made a part hereof.
S. It is necessary that a guardian of the person and
property be appointed for the incompetent.
6. The approximate value and description of the incom-
petent's property are: VALUE -- $36,000.00
