Mr. Ba~~*ett is a 71 year old white man who has
been at the Abbie3eAn Russell Care Center since August
6th 1980.
PRFSENTING .PRORLEMS;
: The psychiatric evaluation was requeste~ by Dr.
Batsche in order to ~et an opinion as to whether Mr.
Baggett is competent to handle his affairs. A review
+ of his records at the NursinF Home indicate that patient
, has a history of stroke and hematuria. He also carriQs
diagnoses of Parkinson's Disease. Atherosclerotic Heart
Disease and Neurogenic Bladder. According to the nuraea
his physical condition has detriorated over the last

Patient is a 71 year old white~man who looke his
stated a~e. He was seen layin~ down in his bed. He
seemed to be ~hysically quite weak and had difficulty
turnin~ from side to side in hie bed. He has left=
sided weakness. His sneech is slurred and difficult
to understand. He is also quite soft spoken. He
was oriented to nerson. He stRted that he wA8 in
an "American Hos~+ital" in Fort Pierce, Florida. Ne
was completelv dieoriented as to time. He atated
that it was Mxrch or April of 1901. When specifically
asked he stated that we had Christmas 3ust a few days ago.
