AFFIDAVIT OF STEPNEN D. FROMANG
I, Stephen D. Fromang, declare as follows:
That I am a resident of Indian River County, Florida; I am over
the age of 21 years; I am an attorney duly licensed to practice
law in the State of Florida; and I am the attorney of record for
Cohesive Risk Management, Inc.

On~anuary 20, 1981, I received in the U. S. Mail, from
Dae Lantz, Jr., the original promissory note and the original
assi~nment of promissory note both dated December 9, 1.966. Said
original promissory note is in the amount of $15,000.00 and was
executed by Barry Dayl to Dae C. Lantz and saTd original promissory
note was witnessed by Addie Coleman. The said original assignment
of promissory note was assigned to Dae Lantz, Jr. and executed by
Dae C. Lantz. Said original promissory note and said original
assignment of promissory note is attached hereto and made a
part of this Affidavit.
I certify under penalty of perjury that the for~goinq is true and
correct.
