PETITION FOR APPOINTMENT OF GUARDIAN
Tlie undersigned Petitioner alleges:
1. Petitioner's residence is Route 8, Box SOS-C, Fort
Pierce, Florida 33450, and Petitioner's post office address is
the same.
2. ALISA ERICA RAMPY is a minor whose date of birth
was April 17, 1972, who is 8 years of age, and whose Social
Security Number is not known. The residence of the minor is '
Route 8, Box 505-C, Fort Pierce, Florida 33450, and the post
office address.of the minor is the same. ~
3. The nature of the minor's incapacity is minority.
4. It is necessary that a Guardian of the property be
appointed for the minor. '
S. The approximate value and description of the
minor's property are:
All assets from the Estate of the
minor's father, CHARLES DOUGLAS
RAMPY, deceased, consisting of
cash in the approximate amount of: $6,000.00
6. The names and addresses of the persons most closely
related to the minor are:
