RECEIVE PROPERTY AS PAYMENT OF GUARDIANS FEE-' a a. '

l. Petitioner, whose address is 19 Pine Boulevard, Lakewood,
New Jersey 08701, was duly appointed the guardian of the property of
Claire D. Reichert, Incompetent, on December 19, 1975, by the Ocean
County Surrogate Court, Ocean County, New Jersey. The Petitioner,
was discharged by said court on ~ugust 24, 1979.
2. The incompetent is a resident of Ocean County, New Jersey,
and is presently located at the Medicenter, Lakewood, New Jersey.
3. On August 24, 1979, Judge Robert H. Doherty, Jr., of the
Superior Court of New Jersey Law Division, Ocean County Probate
discharged Petitioner as guardian of the incompetent, and further
ordered that the real property owned by ~the incompetent in Florida
should be distributed to said petitioner as payment for her acting
as guardian for said incompetent. A certified copy of the proceedings
in the matter of Claire D. Reichert, a mentally incompetent person,
is attached hereto and by reference made a part hereof.
4. Petitioner desires to receive title to the following
described property located in Florida, belonging to the ward:
Lots 13 and 14, Block 1372, PORT ST. LUCIE
SECTION 14, a subdivision according to the
plat thereof, recorded in Plat Book 13, page
5 of the public records of St. Lucie County,
Florida. ~
Value: 53,400.00 '
Lot 24, Block 2910 of PORT CHARLOTTE SUBDIVISION
Section 49, a Subdivision according to the Plat
~ thereof, recorded in Plat Book 5, Page 63A of
the public records of Charlotte County, Florida.
Value: $1,200.00 •
5. There is no indebtedness existing in Florida against the
ward.
