AFFIDAVIT UNDER FICTITIOUS NAME STATUTE
The undersigned, under oath, say that they are engaged
in a business enterprise under the fictitious name of INDIAN
RIVER HELICOPTER SERVICE and their principal place of business
is located at the St. Lucie County Airport, Fort Pierce, St.
Lucie County, Florida. Those interested in said enterprise
and the extent of the interest of each is as follows:
