following doctors to make arrangements for the payment of ine~ical
bills owing to them for services rendered to the minor chil~ of
the parties: NAME BALANCE
Dr. Alfredo Gonzales 100.00
Drs. Cambron, Berg & Mallonee 65.00
Lawnwood Medical Center 39.10
Dr. Jack L. McGowan 52.00
Ra~iology Associates 45.00
~ Scott Medical Laboratory 20.00
The Husband is ordered to make suitable arrangements
with each of the said doctors or medical service representatives
for payment of the above-described unpaid balances owing for services
rendered to the minor child of the parties under such circumstances
as will allow the former Wife to continue to have each of the above-
described doctors and medicai services render future medical services
or care to the minor child of the parties as needed.
3. The former Husband shall continue to have reasonable
visitation rights with the minor child of the parties, after giv.ing
notice to the former Wife of his intent to visit, but such visitation
shall be confined to visitation with the minor child of the parties
and must not result in violation of the restraining order previously
entered herein.
4. A certified copy of this order shall be sent to the
following address represented to this Court as being the mailing
address of the former husband: 1115 Granada Street, Ft. PIp~CP~
Florida 33450
