BEFORE ME, a Notary Public, this date appeared

ANGELA FABRIZIO, to~me well known, who, being by me first duly
sworn, deposes and says as follows:
1. That I am the surviving spouse of FRANK F.ABRIZIO.
2. That my said spouse and I, as_ an estate by the
entirety, heretofore acquired title to the following St. Lucie

Lot 15, Block 1970, of PORT ST. LUCIE SECTION l9,
a Subdivision according to the Plat thereof,
recorded in Plat Book 13, Page 19, of the
Public Records o~ St. Lucie County, Florida,
3. That my said spouse and I were lawfully married
to each other prior to the acquisition of title as aforesaid,
that we remained married to each other, without an intervening
divorce, up to the time of the demise of my said spouse.
4. That my said spouse died on February 12, 1981,
at Port Charlotte, Charlotte County, Florida, and at the ti~ne of
death was a resident of Charlotte County, Florida.
5. That a copy of the death certificate of said
decedent is attached hereto and made a part hereof.
