PETITION FOR aPPOINTTiENT Or GUARDIAN
The ~indersigned Petitioner alleges:
l. Petitioner's residence is RD ~2, Americus, Georgia
2, i~'ALTER DF:Cfi is an incompetent ~~ho is 86 years of age
whose residence address is 4809 Palm Drive, Ft. Pierce, St. Lucie
County, Florida.
3. The nature of the incompetent's incapacity is organic
brain syndrome, severe cerebral ati•oph~•, bilateral carotid disease,
sclerosis of aorta, and inability to care for himself, as adjudged
by the Circuit Court in St. Lucie County, Florida on Atarch ~~
1981.
~1. It is necessar.?• that a guardian of the pei-son and
property be appointed for the incompetent.
5. The approximate value and description of the incompe-
tent's property are: approximatel}~ $1,400.00 in a joint checking
account with his 1ti'ife, NELEN DECH in the Sun Bank of St. Lucie
Count}~, and a joint savings account with the 1~~ife, HELE~I DECH, in
the amount of approximately ^~13,000.00 in Fii-st I~ederal Savings and
Loan Association of rt. Pierce. The incompetent also recei~~es a
monthly income of approximately Sb50.00 from the Social Security
Administration, >ai~ amount Ueing in conjtinction with his t~'ife,
f1ELEN DECH, Joint home located at 4809 Palm Dr., Ft. Pierce.
