mother, PATRICIA TAYLOR WARD, has waived her right to serve as Personal
Representative.
6. The nature and approximate value of the assets in this
estate are none. The sole purpose for the estate administration is to
pursue a wrongful death action based ~n a medical malpractice suit
against Lawnwood Medical Center.
7. After the exercise of reasonable diligen~e, Petitioner is
unaware af any unrevokecl wills ar codi~ils of decedent.
8. Petitioner requests the waiving of the posting of any bond
or surety thereon, inasmuch as there are no probate assets in the estate.
Petitioner requests that JAMES WARD be appointed Personal Rep-
resentative of the estate of the dececient.
Under penalties of perjLry, 7 declare that I have read the
